The BMI Threshold
The FDA-approved criteria for GLP-1 medications prescribed for weight management generally follow two tracks:
- BMI of 30 or higher — You may qualify for a GLP-1 prescription based on BMI alone. This is the clinical threshold for obesity.
- BMI of 27 to 29.9 — You may qualify if you also have at least one weight-related health condition, such as type 2 diabetes, high blood pressure, high cholesterol, or obstructive sleep apnea.
Your BMI is calculated from your height and weight. Many online calculators can give you a quick estimate, but your doctor will confirm this during your evaluation.
What Conditions Qualify?
If your BMI falls between 27 and 29.9, the following conditions commonly qualify you for GLP-1 treatment:
- Type 2 diabetes — Some GLP-1s (like semaglutide, marketed as Ozempic) are separately approved for blood sugar management
- Hypertension — High blood pressure, whether controlled with medication or not
- Dyslipidemia — Elevated cholesterol or triglyceride levels
- Obstructive sleep apnea — Diagnosed through a sleep study
- Cardiovascular disease — History of heart attack, stroke, or established heart disease
Who Doesn't Qualify?
Certain conditions are listed as contraindications on GLP-1 medication labels. Your provider will screen for these:
- Personal or family history of medullary thyroid carcinoma (MTC) — This is a contraindication on all GLP-1 receptor agonist labels
- Multiple endocrine neoplasia syndrome type 2 (MEN2) — A rare genetic condition
- Pregnancy or breastfeeding — GLP-1 medications are not approved for use during pregnancy
Some conditions are evaluated case-by-case rather than being absolute disqualifiers. A history of pancreatitis, for example, may require additional monitoring but doesn't automatically rule you out.
What About Insurance?
Insurance coverage for GLP-1 weight loss medications varies significantly. Some plans cover brand-name medications like Wegovy or Zepbound for qualifying patients. Many don't.
