Here's a side of major weight loss almost nobody discusses at prescription time: bone density declines with substantial weight loss, by essentially any method. It's thoroughly documented in the bariatric surgery literature, it appears in diet-induced loss, and body-composition data from GLP-1 trials shows the same pattern. It is not unique to these medications — but the medications produce surgical-scale losses at population scale, which makes the skeleton's response newly relevant to millions of people.
Why losing weight costs bone
- Bone follows load. Your skeleton continuously remodels to match mechanical demand. Carrying less weight is, mechanically, a lighter training program for your bones — and they economize accordingly.
- Fat tissue talks to bone. Adipose tissue produces hormones (including estrogen, via conversion) that support bone maintenance; shrinking it changes the signaling environment.

For these groups, a baseline DEXA scan around the start of major intentional weight loss — with a follow-up a year or two later — is a reasonable, guideline-consistent conversation to have. For young, low-risk patients, awareness plus the protective habits below is generally sufficient.
The protection playbook
1. Load the skeleton on purpose
Bone keeps what it's forced to use. Resistance training and weight-bearing impact activity (brisk walking, stairs, hiking, jumping-adjacent work as appropriate) directly signal bone to maintain itself — and conveniently, it's the same prescription that protects muscle. Swimming and cycling are excellent exercise but poor bone loaders; if they're your mainstays, add something that fights gravity.
2. Feed the remodel
Calcium (food-first: dairy, fortified alternatives, canned fish with bones, greens; supplement the gap if intake is genuinely low), vitamin D (test and dose with your clinician — deficiency is common and silently expensive here), and adequate protein, which is bone matrix as much as it is muscle.
