The Oral GLP-1 Revolution: A Doctor's Guide to the Pill vs. the Shot in Summer 2026

The Oral GLP-1 Landscape Changed Fast

Six months ago, every GLP-1 medication for weight loss required a weekly injection. Today, patients have two FDA-approved oral options, and the clinical conversation has fundamentally shifted. For the first time, your doctor can offer you a GLP-1 pill โ€” and for some patients, that changes everything.

Wegovy Pill (Oral Semaglutide 25mg)

FDA-approved December 2025, launched January 2026. This is the same molecule as injectable Wegovy โ€” semaglutide โ€” reformulated into a once-daily tablet. In the OASIS 4 trial, patients who took the pill as directed lost an average of 16.6% of their body weight at 64 weeks (about 14% in the intention-to-treat analysis). That is comparable to the injectable version's performance in earlier STEP trials.

The catch: oral semaglutide must be taken on an empty stomach with no more than 4 ounces of plain water, and you must wait at least 30 minutes before eating, drinking, or taking other medications. For some patients, this daily ritual is a minor inconvenience. For others โ€” especially those with busy morning routines or multiple medications โ€” it is a dealbreaker.

Pricing starts at $149/month for the 1.5mg starting dose (cash-pay), with the highest dose at $299/month. Commercially insured patients may pay as little as $25/month with savings programs.

Orforglipron (Eli Lilly)

FDA-approved April 1, 2026. This is different. Orforglipron is not a peptide like semaglutide โ€” it is a small-molecule GLP-1 receptor agonist, which means it can be taken without any food, water, or timing restrictions. You take it once daily, whenever you want, with or without meals. For patients who tried oral semaglutide and found the fasting requirement impractical, orforglipron removes that barrier entirely.

The weight loss is more modest: the ATTAIN-1 trial showed dose-dependent losses of 7.5% to 11.2% at 72 weeks, compared to 2.1% with placebo. Lower than semaglutide's numbers, but still clinically meaningful โ€” and the convenience factor may improve real-world adherence enough to close that gap.

A key advantage: the ATTAIN-MAINTAIN trial showed orforglipron can maintain weight loss in patients who switch from injectable GLP-1s. Average weight change was just -0.1 kg after switching from semaglutide, versus +9.4 kg for patients switched to placebo.
